Permanent Residence For EEA members?

As a Swedish citizen who lived and worked in UK for about 5 years and 6 months

1/ What application form i should to fill to get a permanent residence in UK, is't EEA1 or EEA3 please?

2/ As evidence of residence in the UK for over 5 years, what Proof of residence i should to send with the application? Shall i send all utility bills&statments what it cover 5 years such as ( gas bills, electricy bills,water bills, bank statmetns, mortgage statments,BT bills ) i think it will be too much to send all of them.

3/ Can i send my European ID card instead of my passport?

4/ How long will take till i will get an answer please?
