How to stop identity thief (unusual situation)?

How can I stop a family member from using my SS# and other info to impersonate me and break into my private accounts?
She has copy of my birth certificate, ss card, driver's license #, expired passport, old medical records, loan account info.
She does NOT steal $. But makes changes to my accounts. She won't ever stop.
