How to transfer a house into my spouses name?

Hi there,
My wife and I own our home, outright, no mortgage, no loans, no debt, etc. The house is in both our names. I have recently become ill and I want the house transferred into her name and her name only. There will be no exchange of money, etc. How can I go about doing this?

Added (1). This is not being done out of fear of medical debts or anything. I just want the house to be in her name, that is all.
